Registration

The following documents must be completed, produced and verified upon registration:

- ECDA FORM 1 (Registration)

- Acknowledgement of School Terms and Conditions

- Parent’s and Additional Information of Child

- Parent’s Agreement on PDPA

- Child’s Birth certificate / Passport

- Child’s Immunization record (with all mandatory immunizations in Singapore)

- NRIC / Passports of parents / legal guardian

- Main Applicant’s Employment details (Mother / Single Father)
• NOA/ACRA/Consent for ECDA to verify information with CPF Board, IRAS and ICA.

- If applying for Additional Subsidy under Per Capita Income
• Photocopies of NRIC of all family members residing in the same address

- 1 recent family photo showing:
• Parent(s) + child;
• Any authorised personnel (above 18yrs) to fetch the child

- CDA details (for parents who wish to use CDA to pay school fees)
